Hot nose sign – an indirect sign of brain death on nuclear medicine cerebral perfusion studies. Absence of internal carotid artery flow leads to increased external carotid artery flow and subsequent increased perfusion in the nasal region. Although the sign is cute, it is not of any real diagnostic value as the medico-legal importance of brain death is such that direct assessment of cerebral perfusion is required.

Double duct sign – simultaneous dilatation of the common bile…

Double duct sign – simultaneous dilatation of the common bile duct (red arrow) and pancreatic duct (yellow arrow). The two most common causes of the double duct sign are carcinoma of the head of the pancreas and duodenal ampullary tumours (blue arrow). Because the causative tumours are often small, recognition of the sign can be critical to making a diagnosis. It may be observed on almost all imaging modalities including MRI, CT, ultrasound and ERCP.

Empty delta sign – of dural venous sinus thrombosis within…

Empty delta sign – of dural venous sinus thrombosis within the superior sagittal sinus on contrast enhanced CT. The thrombus creates a filling defect within the sinus outlined by an enhancing triangular rim. Of course calling it the ‘empty triangle sign’ would be too simple and so quite rightly the Greek letter delta (Δ) is used to add an air of mystique.
